​​​You can request transcripts or other records from the Total Homeschool Program by email (preferred), phone, mail, or fax. Requests should include the student's full name, date of birth, and year of graduation. Please give us instructions as to where and how to send it (mail, fax, or email).

Timing

Typically, we send out a transcript within 10 days of receiving the request. Please be aware of deadlines and make your request early. Always inform us of any deadlines you’re facing.

What We Need

Be sure to include the following information:

• Your name and your relationship to student

• Full name of student (including middle and maiden name, if applicable). Let us know if the current last name should be added to the transcript.

• Date of birth

• Year of graduation (or last date of enrollment)

• Name and address/email/fax of location to send transcript

• Your phone number or email address (A counselor may need to contact you before sending the transcript.)
