DMV Form (SF-1010)

Students going to the DMV for a learner’s permit or driver’s license need an SF-1010 (Certification of Compulsory School Attendance) form from their school (Gateway). It's valid for 30 days, but usually extended through the summer when school is out. To receive the form, students must be enrolled and making satisfactory progress in school, which we will confirm by checking the grades and/or calling the parents.

 

You can request an SF-1010 form by email (preferred), phone, mail, or fax. Many DMV offices will accept a printed copy of an emailed SF-1010 form. Some, however, require an original by postal mail. Contact your local DMV to be sure.

Transcripts and Other Records

​​​You can request transcripts or other records from Gateway Homeschool by email (preferred), phone, mail, or fax.

Timing

Typically, we send out a transcript within 10 days of receiving the request. Please be aware of deadlines and make your request early. Always inform us of any deadlines you’re facing.

What We Need

Be sure to include the following information:

• Your name and your relationship to the student

• Full name of the student (including middle and maiden name, if applicable). Let us know if the current last name should be added to the transcript.

• Date of birth

• Year of graduation (or last date of enrollment)

• Name and address/email/fax of location to send transcript

• Your phone number or email address (A counselor may need to contact you before sending the transcript.)

Diploma Reprint

If you would like a reprint of your diploma (on parchment paper), there's a fee of $25. Click the button below to submit payment and give us your information. Send us an email with the address where the diploma should be sent if PayPal doesn't have the correct information.
